What to bring and how to prepare for your first visit.
Save time at your first visit
Arriving with your paperwork already completed helps your first evaluation start on time and gives your therapist more time to focus on you. You can complete your patient forms online through our secure, HIPAA-compliant portal before your visit. If you are not able to complete forms ahead of time, arrive 15 minutes early and we will get you set up in the office.

What to bring with your forms
- Your physical therapy referral, if you have one (a referral is not always needed)
- Your insurance card
- Your claim number and case manager's contact information, if covered by workers' compensation
- Payment information
